- Submit a contact, appointment, subscription, or other website form;

1. Information We Collect
Information You Provide
Depending on how you interact with us, you may provide:

- Appointment preferences, service interests, and inquiry details;
- Email, SMS, and telephone communication preferences and consent records;
- The content and metadata of form submissions, emails, text messages, and other communications; and
- Other information you voluntarily provide.
Please provide accurate and current information. General website forms should not be used to submit Social Security numbers, payment-card information, medical records, diagnoses, prescriptions, photographs of medical conditions, or other sensitive health information.

We use LeadConnector CRM, including its forms and communication tools, to collect and manage:

- Telephone numbers;

We disclose information to companies that provide hosting, CRM, communications, analytics, advertising measurement, security, technical support, and related services on our behalf. These companies include Hostinger, Google, Meta, Microsoft, and LeadConnector.
Advertising and Analytics Partners

9. Medical and Health Information
Inject SkinCare is a Nurse Practitioner-owned medical spa. Treatments are provided only after a clinical evaluation and under a medical plan of care.
This Privacy Policy covers our public website, online forms, marketing activities, and general communications. It is not a Notice of Privacy Practices for medical records.
Information collected in a clinical setting may be governed by separate consent forms, medical-record practices, and legal requirements not described in this Privacy Policy.
Do not use general website forms, marketing emails, or text messages for emergencies or to submit medical records or detailed health information.
If you believe you are experiencing a medical emergency, call 911 or seek appropriate emergency care.
10. Information From Minors
Our website is not directed to children under 13, and our marketing and analytics tools are intended for adults.
We do not knowingly collect personal information online from children under 13. If you are under 18, do not submit a form or subscribe without the involvement and consent of a parent or legal guardian.

Inject SkinCare operates in the United States.
